I certify that the above information is true and complete, and I authorize Logos to conduct a review of my character and ability to perform the job for which I am applying. I understand that in the review process, reports may be solicited from previous employers and other references, but no attempt will be made to contact my present employer, unless specifically authorized by me to do so. I also understand that for this certification to remain valid, the renewal process must be completed on or before the anniversary date of the certificate annually.
